Terms and Conditions of Use for MyChart

Introduction

El Rio Health is pleased to extend the use of MyChart to its patients as a convenient and secure way to access your El Rio Health medical and dental information and communicate with your provider’s team. MyChart is an internet application that allows patients to review their medical information, receive certain laboratory results, access their care team, make appointments and make personal notes. Patients may also access educational materials. The electronic nature of MyChart requires patient/user responsibility and as such, you must read and accept the following Terms and Conditions to access or request a MyChart account. You may decline to use MyChart at any time, but our reason for making this service available is to empower you as a patient to take an active role in your health care.

Terms and Conditions

MyChart and related services are provided by El Rio Health to its patient’s subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of MyChart will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit the application. El Rio Health reserves the right to modify the access to MyChart in the event you choose not to comply with these terms and conditions.

Privacy

Your privacy is very important to us. Please review El Rio Health’s Notice of Privacy Practices for a description of how we use, share and protect your confidential health information. If you have information you want shared only with your provider, you should contact your provider directly and not communicate through MyChart.

You control who has access to the information in MyChart. When utilizing MyChart, you will be prompted to follow the directions to log into MyChart using a username and password that you create. You are responsible for deciding who, if anyone, you share your log-in information with.

Proxy Access/Access by Others in MyChart

You may give access to your MyChart account by granting access to MyChart to individuals known as “a Proxy”. If you have previously authorized another person (a “Proxy”) to have access to your MyChart account, your Proxy will have access to the information contained in your MyChart account. You may revoke Proxy access at any time by removing the Proxy through your MyChart account.

Secure Communications

All communications between you and El Rio Health using MyChart occur over a secure connection. All of the confidential information available to you in MyChart is protected and securely maintained by El Rio Health.

E-mail Privacy

As a user of MyChart you should be aware that you will be notified via e-mail when there is new medical information to be viewed on MyChart. This means that any person with access to your e-mail will be able to see this notification. This could include your family members, employer or anyone else who can access your e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that you would not want others to know. Thus, you should take this into account when providing an e-mail address or using MyChart.

Incomplete Medical Record

The portion of your medical record available in MyChart may not be your entire medical record. The scope of what is provided in your MyChart account is at the discretion of El Rio Health.

Non-Urgent Matters

MyChart is not a substitute for medical care and should not be used for emergencies or urgent matters. In an emergency or urgent situation, please contact your nurse or physician directly.

Restrictions on Use and Ownership

All pages within MyChart and any material made available for download are the property of El Rio Health and/or its subsidiaries and affiliates. The content on MyChart is provided for El Rio Health and its patients only. The words “El Rio Health” and its associated corporate logo are the trademarks and service marks of El Rio Health. All other trademarks, service marks, and logos used in this MyChart, including but not limited to MyChart and Epic are the trademarks, service marks, or logos of their respective owners. Nothing in MyChart should be construed as granting any license or right to use any trademark, service mark, or logo displayed on MyChart without the express permission of El Rio Health or the third party that may own the trademark, service mark, or logo.

Accuracy and Integrity of Information

El Rio Health attempts to ensure the integrity of MyChart; however, it makes no guarantees as to the accuracy of the information on MyChart. It is possible that MyChart could include typographical errors, inaccuracies or other errors. In the event you discover an inaccuracy, please inform El Rio Health’s Health Information Management Department so that it can be corrected. Information contained in MyChart may be changed or updated from time to time without notice.

El Rio Health makes no representations whatsoever about any other web site that you may access through this site or any entity that may post its advertisements on this Site. When you access a non-El Rio Health site, please be advised that it is independent from El Rio Health, and that El Rio Health has no control over the content on that web site. Termination of Access

Failure to comply with these Terms and Conditions and/or abuse of the MyChart application, such as excessive messaging or recording others without their permission, may cause your MyChart access to be revoked without prior notice. El Rio Health may terminate your access to MyChart at any time, without notice and with or without cause. If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance.
